Valley Primary Care Clinics Sustain Recognition as NCQA Patient-Centered Medical Homes

We are excited to share that our primary care clinics have successfully sustained their recognition as National Committee for Quality Assurance Patient-Centered Medical Homes (PCMH)! Each year our primary care clinics must prove they meet the 40 core criteria and 25 elective criteria to maintain their distinction of being Patient-Centered Medical Homes; we have achieved this recognition every year since our initial application in 2018. “I want to express how incredibly proud I am of all the staff in the primary care clinics for their unwavering dedication to serving patients and providing them with a true sense of care and belonging.  Thank you for always creating a welcoming and supportive environment that feels like home for our patients,” said Kris Nakanishi, Director of Primary Care for Valley’s Clinic Network.

NCQA’s Patient-Centered Medical Home program is the most widely adopted PCMH evaluation program in the country, with more than 10,000 practices holding the recognition and demonstrating their dedication to putting patients at the forefront of care. When a clinic is recognized as an NCQA PCMH, it is an indicator that they have a commitment to build strong relationships between patients and care teams, demonstrate continuous quality improvement, strive for the best patient experience and increased staff satisfaction, all while working to reduce healthcare costs.

“Each year, the landscape of Primary Care offers more challenges than the previous year. I continue to be impressed that our providers perform the right work every day, and this dedication to our patients makes it possible to maintain our PCMH recognition,” shared Dr. Shannon Markegard, Associate Medical Director of Primary Care. This effort by our physicians and advanced practice providers benefits our patients in several ways. Patient care is less fragmented, with increased focus on team-based care, communication, and proactively coordinating care. Additionally, we can more fully engage our patients with chronic conditions in care management. Here at Valley Medical Center, we as so proud of the work our staff do every day to care for our community like family!
