Natalie Ervin, ARNP, Lake Sawyer Clinic
Natalie has more than a decade of experience working in multiple specialties. Recently she practiced in a busy family practice clinic where she cared for patients of all ages; providing care for women, children, acute and chronic conditions, hormone replacement, with a focus on preventive measures. She has a special interest in educating her patients and discussing their overall health and wellness so they can attain the highest quality of life.

Maiah Hall, ARNP, Valley Women’s Healthcare
Maiah’s scope of practice is vast, including caring for pregnant and postpartum people, caring for newborns for the first month of life, evaluating concerns for gynecologic issues including infertility concerns, providing contraceptive services, managing menopausal concerns, and preventive wellness visits. She is dedicated to providing individualized, inclusive, and accessible healthcare to all, including managing gender-affirming hormone therapy for trans and non-binary folks and providing family planning services.

Leigh Kalaman, ARNP, DNP, Highlands Clinic
Leigh approaches her role as a PCP as a partnership with her patients, working with them to reach their healthcare goals. She enjoys caring for patients across the lifespan with a special interest in women’s health, trauma-informed care, and gender-affirming care.

Reza Masoomi, MD, Cardiology Clinic
Dr. Masoomi is an interventional cardiologist who is board-certified in cardiology, echocardiography, internal medicine, and interventional cardiology. He is dedicated to performing advanced coronary interventions using cutting-edge technology, always prioritizing both patient safety and exceptional outcomes.

Manu Mooker, ARNP, Internal Medicine Clinic
Manu cares for patients as a member of her healthcare team, meeting them where they are and understanding the barriers and motivators to their health. Her passion lies in educating patients and helping them make informed decisions with the goal of maintaining health at the most optimal level. As a board-certified adult-gerontology nurse practitioner, Manu enjoys working with adults of all ages in managing both acute and chronic conditions.

Nam Pham, DO, Infectious Disease Clinic
Dr. Pham is an infectious diseases physician committed to establishing strong relationships with his patients in order to provide comprehensive care tailored to each individual’s circumstances. He has a particular interest in treating a wide range of acute and chronic infections, such as HIV, bone and joint infections, endocarditis, and intra-abdominal infections. Dr. Pham is an advocate for the LGBTQIA+ community and prescribes pre-exposure prophylaxis (PrEP) for patients.

Joshua Trierweiler, MD, Urology Clinic
Dr. Trierweiler is dedicated to providing patient-centered care across the spectrum of urologic conditions. His goal is to know his patients on an individual level, and to help them make the best healthcare decisions for their personal goals and circumstances. He has experience training patients with kidney stones, kidney cancer, bladder cancer, prostate cancer, enlarged prostate, erectile dysfunction, and urinary incontinence. He also has experience with minimally invasive and robotic surgery.

Alex Wang, MD, Fairwood Clinic and Sports Medicine Clinic
Dr. Wang is a primary care family medicine and sports medicine physician who loves taking care of patients of all ages and backgrounds. He takes pride in building strong, collaborative relationships with his patients and spending the time to truly get to know them. Dr. Wang is passionate about helping patients navigate the healthcare system, empowering them with the information they need to understand their health and make healthy decisions.
