Cheers to a Healthy 2025: A Friendly Reminder to Schedule Your Annual Health Maintenance Checks

It’s almost time to ring in the new year! It’s the perfect time to prioritize your health. Make sure these essential health appointments are on your calendar for 2025:

1. Schedule Your Yearly Checkup with Your PCP
Getting a yearly checkup (annual physical) is one of the most important steps you can take to maintain your health. During this visit, your primary care provider (PCP) will review your medical history and medications; check your height, weight, and blood pressure; listen to your heart and lungs; and perform any bloodwork or other necessary tests and exams needed, depending on your age and health history.

2. Stay on Track with Preventive Health Screenings and Vaccinations
Preventive care helps detect health issues early. Your PCP will screen for diabetes, high blood pressure, heart disease, cancer, and a variety of other diseases and conditions. Your PCP also evaluates your need for various vaccinations.

Your PCP may also recommend screenings available at other Valley clinics. Some clinics you may have appointments at and screenings your PCP may order include:

3. Don’t Miss Your Annual Medicare Wellness Visit
If you’re covered by Medicare Part B or have a Medicare supplemental plan, don’t forget your Annual Wellness Visit! This appointment with your PCP focuses on prevention, rather than treating current health issues. Together, you and your provider will:

  • Create a personalized health plan
  • Conduct a health risk assessment
  • Review advance care planning
  • Identify any needed screenings or preventive care services

This visit is free for those who qualify under Medicare’s guidelines. Learn more about the Medical Wellness Visit by viewing our flyer or watching this DocTalk video.

Scheduling these visits ahead of time ensures you stay on track with any ongoing treatment or monitoring you need for specific health conditions. Reach out to your specialist’s office or use MyChart to view and manage upcoming appointments.
